Pediatric Foot Care

Do you notice your child complaining of foot pain or unable to keep up with other children? More than likely your kid is suffering from one of the common conditions that often affect their feet such as flat feet, heel pain, ingrown nails, warts, or shin splints. These complaints should be taken seriously and treated to prevent a small problem from becoming a major one! If you notice your child changing how they walk or complaining of pain in their feet, ankles or knees then more than likely they could be suffering from flat feet. Flat feet can put stresses on the joints and tendons which can lead to a lot of pain and fatigue in the foot and legs. This can be treated using custom orthotics which help put the foot in a proper position so it doesn’t have to work as hard. Kids can have heel pain just like adults do, but the reason behind it can be much different. Heel pain can sometimes be due to issues with the child’s growth plate and can be a serious condition. Children with heel pain should be evaluated and treated to ensure that there are no long-term effects. Believe it or not ingrown nails can be a very common problem for children. Tight shoe gear, athletics or improper trimming of nails can lead to this condition. Ingrown nails should be treated promptly to prevent infection. If this is an ongoing issue, it can be treated in the office to remove the offending nail border forever! Warts are another common problem amongst kids. They can appear out of nowhere and be very painful and very hard to treat! Due to the thickness of the plantar skin, warts can be very stubborn and freezing alone is not enough to resolve the issue. Removing warts often requires a combination of topical medication and debridement for full resolution. Shin splints is an ailment that children can acquire with increased activity. This condition occurs when there is excessive force in the muscles that sit close to the bone leading to inflammation and pain. Shin splints should be evaluated and treated with anti-inflammatories and orthotics to reduce pain as children continue to develop. Orthotics

Orthotics are footwear inserts worn to provide foot support and alleviate the discomfort, pressure, pain, imbalance, and other symptoms caused by foot and ankle deformities, injuries, misalignments, and degenerative diseases. Orthotics, or orthoses, can be custom-molded and prescribed by a podiatrist, or they can be purchased over-the-counter in a pharmacy or sporting goods store. Prescription orthotics are custom-fitted and molded to address a patient’s specific foot structure, gait, and ailment, whereas over-the-counter inserts add some support and treat minor discomfort. Doctor-prescribed orthotics are more durable and provide more support and correction that over-the-counter versions. Prescription orthotics are fitted and constructed using innovative technology that scans the patient’s foot, providing podiatrists and orthotic laboratories with a computerized analysis of the foot that addresses the patient’s unique foot structure and walking style. They can also be crafted from a mold generated from a plaster cast of the foot. Orthotics are used to treat a range of foot and ankle conditions, including but not limited to bunions, corns, calluses, high arches, flatfoot, in-toeing/ out-toeing, big toe ailments, hammertoe, claw toe, forefoot pain, neuropathic ulcerations, and heel pain. Common types of orthotics are arch supports, insoles, heel pads, and foot cushions. The two main classifications of orthotics are accommodative and functional; accommodative orthotics are softer and provide extra support and cushioning for improved comfort and reduced distress on the problem area of the foot, whereas functional orthotics are more rigid and treat problems and pain associated with foot and ankle motion. Shapes, sizes, and materials used to construct orthotics vary, depending on the needs of the patient.

Bunions

A bunion is a deformity of the big toe joint that results from bone misalignment or repositioning at the joint. Although bunions occur most frequently at the base of the big toe, they can also arise on the outside of the foot at the base of the small toe. When the joint at the base of the big toe becomes enlarged, friction and pressure are created as it painfully rubs against footwear. Eventually, the big toe may press inward against the smaller toes, even overlapping as far as the third toe in some cases, a condition called hallux valgus. With some bunions, the big toe also rotates or twists inward, which is called hallux abducto valgus. What Are Risk Factors that Can Cause Bunions? Overpronation, flat feet, or other neuromuscular problems that affect the feet Tight-fitting, narrow-toed, or high-heeled footwear Recurring stress to the foot Previous foot injuries Congenital defects Arthritis What Are Symptoms of Bunions? Chronic pain and discomfort when walking, especially as the bunion becomes larger and rubs against footwear Swelling, redness, and tenderness at the site Thickened skin on the bottom of the affected area How Are Bunions Treated? Apply ice Rest the area and avoid motions that apply pressure to the affected joint Wear loose-fitting, non-constricting footwear that does not put pressure on the bunion Insert orthotics into footwear to provide stability and correct positioning Take over-the-counter anti-inflammatory medications on a temporary basis for pain and inflammation (ask your doctor first) Stretch the area with specific exercises to keep the joint mobile Splint the area during sleep to encourage proper joint alignment, which is preferred for younger people suffering from bunions because their bones are still developing Receive a cortisone injection to reduce joint inflammation Remove corns or calluses Have a bunionectomy, or surgery to remove the bunion and realign the toe, if necessary Untreated bunions can also lead to other toe deformities, such as hammertoe. Arthritis and bursitis may also develop in the joint area. How Can Bunions Be Prevented? Wear properly fitting, supportive footwear or custom orthotics Seek early, correct diagnosis and treatment to slow or stop bunion development

Heel and Arch Pain

One of the most common foot problems that I see in my office is plantar fasciitis. This is characterized by heel pain first thing in the morning when you get out of bed or after sitting for a period of time and then standing up, and usually the pain is felt at the base of the arch or in the center of the heel. This can be easily cured by a series of cortisone injections, anti-inflammatories, stretching, icing, and custom made arch supports, which are also known as orthotics. Fractures

There are many bones in the human foot, some of which are very small. In fact, almost 25% of the body’s bones are housed in the feet. Thankfully, a fractured, or broken toe or forefoot bone is usually not serious or disabling. However, foot bone fractures are still painful and require proper diagnoses and treatment. Bones are susceptible to two kinds of fractures: stress and general. Stress fractures are small fissures or cracks in the surface of the bone and usually occur in the forefoot, or the area from the mid-foot extending to the toes. General fractures travel into the bone beyond its surface and can be stable or displaced, as well as closed or open. Displaced fractures occur when bone ends no longer stay in proper alignment with one another. With an open fracture, the broken bone pierces through the skin. The fifth metatarsal, or pinky toe, is vulnerable to several types of different fractures. Ankle-twisting injuries can cause avulsion fractures; with this, the tendon that attaches to the fifth metatarsal bone tears, pulling away a tiny piece of the bone. A Jones fracture is a more serious injury occurring at the base of the fifth metatarsal, which can restrict blood flow in this area that already receives less blood flow due to its location on the foot. What Are Some Causes of Fractures? Stress fractures from sudden exercise increases or overuse Jarring jumps or falls Accidents causing hard blows or impacts to the foot Falling objects that crush bones in the foot Twisting hard enough to cause the bones to snap or break What Are Symptoms of a Fracture? Swelling Bruising of the injured area Pain Tenderness and sensitivity to touch Difficulty walking or putting weight on the foot How Is a Fracture Treated? Apply ice to the area and elevate the affected lower leg to lessen swelling Wear a compression wrap, stiff orthopedic shoe, or walking boot to immobilize and protect the foot Keep weight off the foot and rest it as much as possible

Neuroma

Neuromas are benign growths of nerve tissue, or nerve tumors, that form when the nerves are irritated by surrounding tissue rubbing against them. Improper footwear that creates pressure on the foot, as well as foot structure abnormalities and injury to the nerves can cause or worsen neuromas. Symptoms of a neuroma include intense pain, swelling, tingling, numbness, and/ or a burning sensation in the toes and forefoot area. The pain usually intensifies when walking. Neuromas most frequently develop between the third and fourth toes. Morton’s neuroma is a condition where the tissue surrounding the nerves thickens due to constant irritation or pressure, such as from wearing high-heeled or constricting shoes. Treatment for a neuroma may include wearing properly fitting shoes with a roomy toe box, as well as cushioned shoe inserts or pads that alleviate pressure. The foot can be rested, elevated, and iced to relieve discomfort, swelling, and irritation. High heels or narrow-toed shoes that put pressure on the forefoot should be avoided. Since neuromas can worsen, consult a doctor for proper treatment, which may involve cortisone injections and even surgery in more severe cases to remove the neuroma.

Prolotherapy

Prolotherapy is categorized in the medical field as Regenerative Medicine . This specialty medicine is exactly what it sounds like: promoting regeneration of tissue. It involves the use of natural substances that stimulate your body to heal an injury rather than using surgery to repair or replace your injured ligaments or joints. This growing medical spe cialty has been around since the 1950’s and has a huge fan-base with professional athletes since it is agreat alternative to surgery and requires little to no down-time. Advanced Podiatry is one of the few offices in the Tampa Bay area which offers this service to patients and has been using Prolotherapy for over 7 years. Prolotherapy is a treatment offered at Advanced Podiatry and involves a series of specialized injections which stimulate an inflammatory response initiating healing in an area of injury. You may suffer from plantar fasciitis, ankle ligament, tendon injuries or arthritic joints and take medications such as Advil or have attempted steroid injections which may only provide temporary relief. While most patients show positive responses to these standard conservative treatment options, some patients continue to have pain or repeat injuries. When these options fail, most surgeons recommend surgery as the final treatment option. At Advanced Podiatry we believe Prolotherapy is an excellent alternative to steroid injections and surgery! Although surgical services are provided by our doctors, we understand the reluctance that you may have about surgery. Most surgeries require down-time including time off work, and surgery always has a risk of complications afterwards. During your initial consultation, the doctor will perform an exam and evaluate standard conservative treatment options prior to discussion of Prolotherapy, since it is most often used after initial conservative efforts have provided minimal relief. However, Prolotherapy is often used as a first line treatment for those patients who would like to avoid steroids. The treatments are often used in combination with musculoskeletal laser and custom arch supports or orthotics. The treatments are performed in the office. No sedation or anesthesia is required except for use of cold spray and a small amount of local anesthesia to numb the skin in the area of the injection. The Prolotherapy injection consists of a specially prepared mixture of all natural substances including dextrose and an anesthetic solution that is injected into the area of interest (i.e. plantar fascia, ligament or joint). To enhance accuracy, ultrasound is used to visualize guidance of the injection right into the area of interest. Once the Prolotherapy solution has been injected, the body will begin to create an inflammatory response to the solution and sends growth factors and other cells that promote healing to the area.
